  • Hepatitis B
  • MMR
  • Varicella
  • Influenza

Immunizations Records Submit to and Date:

  • Please submit to Dr. Kimberly Cunningham at Kimberly.Cunningham@ihs.gov and  Nancy Shoemake at Nancy.Shoemake@ihs.gov.  Please bring copies of your immunizations with you on your first day of clinic and give them to Dr. Cunningham.

  • Letter of good standing from UC Berkeley
  • Background Check

Documentation Submit to and Date

  • Please contactDr. Kimberly Cunningham at Kimberly.Cunningham@ihs.gov and Nancy Shoemake atNancy.Shoemake@ihs.gov at least three months prior to the start of the rotation.  She will send you a packet of documents to complete and will also schedule you for an orientation during your first week.  Please bring copies of your documents with you on your first day of clinic and give them to Dr. Cunningham.
