Documentation Required

• Copy of your Photo ID (Driver’s License or State ID Card) showing full legal name, DOB, and address.
• Copy of your UC Berkeley HIPAA training certificate of completion. Upon on-boarding at LightHouse, you will receive additional HIPAA information specific to LightHouse and need to sign an attestation that your UCBSO HIPAA training and our LightHouse specific HIPAA information will be applied to their LightHouse services.
• You will receive on-boarding instructions including a review of the relevant portions of the LightHouse employee handbook for topics such as safety, incident reporting, workplace culture, etc. A digital copy of the handbook will be given to you during on-boarding.

Documentation Submit to and Date

• Please submit all documents to Dr. Truyet Tran at ttran@lighthouse-sf.org at least three weeks before the start of the session.
