Refund Procedures
All cancellations will be subject to a $10 cancellation fee and must be made in writing, either by mail, email, or fax. We do not accept phone cancellations. To receive a full refund minus cancellation fees, cancellations must be made by Tuesday, September 17th. Cancellations received between September 18th and September 27th will be refunded 80% minus cancellation fees. Cancellations made after September 27th or No Shows will not be refunded.

Medical Emergencies
Cancellations due to medical emergencies must include a note from your primary or diagnosing physician. To receive a full refund minus cancellation fees, we must receive your cancellation at least 4 days (96 hours) before the first day of the program. Cancellations made within 4 days of the first day of the program, or No Shows, will receive an 80% refund minus cancellation fees.

If original payment was made by credit card, your cancellation refund will be credited to your credit card account.

Program Changes
If you make any changes in the dates of your attendance, you will be charged the fees that are in effect on the date you make the changes. Changes in your registration may be made by mail, email, or fax, and are contingent upon available space. All program changes must be made at least 4 days (96 hours) before the program start.

Featured Speaker: Jorge Cuadros, OD

The Thomas H. Peters Memorial Lecture on Telemedicine will be presented by Dr. Jorge Cuadros, Assistant Clinical Professor and Director of Clinical Infomatics Research at Berkeley Optometry. He is also currently directing UCB Retinal Reading Center for diabetic retinopathy detection. Starting in 1994, Dr. Cuadros developed several programs for remote clinical diagnosis and distance learning, including programs in China, India, and Latin America. EyePACS was developed as an open access system for clinical communication in eye care, and has been used for teleconsultations, retinopathy screening, home care, education, digital grand rounds, and research. He is a member of DICOM’s working group on data interchange for ophthalmic diagnostic information objects, the defacto international standards organization for eye care imaging. He is also a member of Health Level 7, the American Medical Informatics Association, and the American Telemedicine Association. As the past chair of Association of Schools and Colleges of Optometry’s informatics special interest group, Dr. Cuadros led the development of informatics standards for optometry schools.
